Working Papers
Working papers are required for all minors under sixteen years of age. Students may apply for working papers at the Superintendent's office located at:

The following are required for the application:
1. The employer's federal ID number
2. Verification of birth date (birth certificate or license)
3. Student's social security number
4. Parent must come in person to sign
5. Copy of most recent grades
6. Employer's Request Form (If employer does not provide this form, the
Superintendent's office has the form.)
7. Student must come in person to sign
The Superintendent's office is open Monday through Friday, 8:00am-4:00pm.
