Free Flu Clinic

Dear William Allen School Families,

Welcome back to a new school year! This fall, William Allen School will host a FREE school based flu clinic for all students where your child can get an annual flu vaccine. All vaccinations will be given by trained medical staff. This flu clinic is offered in cooperation with the New Hampshire Immunization Program (Division of Public Health Services) and the Strafford County Public Health Network.

Why sign your child up for this year’s flu clinic?
 It is provided at NO COST to your family.
 Your child will be less likely to get the flu and miss school time.
 You won’t have to make a trip to your medical provider.

The best way to protect against the flu and its complications is to get a flu vaccine each year. Anyone can catch the flu, but rates of infection are often highest among children. Some children may suffer from complications if they have other health conditions like asthma, diabetes, or heart disease. The Centers for Disease Control and Prevention (CDC) and the New Hampshire Department of Health and Human Services recommend that everyone six months of age and older get a flu vaccine.

This year we have online registration! The link to register your child is listed below, along with
the details of each school’s clinic. A paper consent form is available to you if you prefer, if you
do not have access to technology, if you plan to opt-out of New Hampshire’s vaccine registry
(NHIIS), or if you need a consent form in another language. To obtain a paper consent form,
please contact your school nurse.

Directions on how to sign you student up can be found at SCPHN.org/fluconsent
Your child’s vaccination information is confidential and protected by state and federal law.
Vaccination information is safely stored in New Hampshire’s Immunization Information System
(NHIIS), which brings together information from different health care providers into a single
permanent record. Your child’s immunization record is available to you at any time. Additional
information is available at dhhs.nh.gov/dphs/immunization/nhiis-public.htm.

You have the right to opt out of NHIIS by completing the opt-out form located here:
dhhs.nh.gov/dphs/immunization/documents/nhiis-opt-out.pdf. This form should be kept with a completed paper consent form. However, if you opt out, you will need to share your child’s immunization records with their medical provider and the school health office to keep their
vaccine information up to date.

Vaccine Information Statements provided from the CDC are available at the following link to help answer your questions: immunize.org/vis/flu_inactive.pdf. You may also direct questions to your school nurse.

Sincerely,
Strafford County Public Health Network Team
